Pretty sure HQ is the product of an abusive childhood who has mental issues with that manifest after treating mentally unstable patients who have an unhealthy affect on het.
Highly recommend the Harley Quinn (2019) series if this is something that you’re interested in - It’s not exactly the plot but the focus is properly on Harley instead of Batman, The Joker, etc. (although it does take place in the DC universe and they play their roles)
There is a female joker, but she went crazy because she was Martha Wayne and Bruce was the one killed in that universe. Thomas Wayne became Batman.
He became Batman with guns
Also went crazy (thus becoming Batman)

There’s an alternate timeline in the DC universe where Bruce Wayne was killed in crime alley. Thomas Wayne became Batman and Martha became the Joker because her mind just broke at the death of her son.
